GENERAL QUESTIONS

The main things you may want to bring are:

  • Clothes which allow the area to be assessed
  • Any medical information that is relevant.
  • Any consent forms that have been filled out in anticipation of treatments.

There is a strip of parking on the road infront of the building. It says for 30mins only.

A little secret though is that the attendant takes a long time to :

  1. Notice your car and,
  2. come back round.

No patient has had a ticket for a 1hr appointment in 5 years. This is at the patient’s individual judgement though.

This estimate will obviously also depend on the complexity of the issue or you as a patient.
 

This is not a problem. However at Rebound Physio we like to be thorough with each issue otherwise and incomplete understanding or less optimal treatment given. This may mean we need an average of three per issue however often if progress is made in one or two and we can move onto the next issue.

This is the use of sterile and single use fine needles inserted into specific muscles which have been assessed to be causing pain or movement dysfunction. It is objectively measured, such as muscle length taken, then post needling measured again with immediate results gained thus allowing its effect to be assessed, a known amount of sessions to be required and no vague outcome which may be apparent with acupuncture. During needling the needles may then be moved gently in and out to release the muscle or just left in place, dependent on client choice.

Dry needling works by:

  1. Touching on the junction between the muscle and its nerve to illicit a response to release the spasm (continued contraction) of that muscle.
  2. Reducing the tightness of connective tissue to allow the normal physiological process to occur (cellular signalling & metabolism etc ), renewed blood flow and the nutrients cells needs to return to healthy states i.e the Oxygen required to create energy (ATP) which is required to release an unnecessarily sustained muscle contraction.

Conditions commonly treated include: Overuse injuries & stiffness from sports i.e. running, boxing, swimming and cycling

To be conscientious and thorough when providing a diagnosis and collaborative in achieving the most effective treatment options for you. Whilst we provide longer sessions, this means more actual physio rather than the rushed meet great / undress of a 30min and whilst more expensive per session this means less are needed.

No. This machine is mainly used to treat tendon issues. These can also be treated with injection which have been shown to be even more effective in certain studies.

INJECTION THERAPY

Unless a returning customer all injection patients will need a prior physio appointment of at assessment to ensure they are on the appropriate path

Steroid is a medication requiring a prescription. We do not currently prescribe at the clinic, although this should be available February 2023, and therefore whilst we are able to administer and guide an injection we cannot provide the steroid.

A letter to the GP explaining the need for a prescription for us to administer is one option and this may also result in the GP offering this themselves hastening your relief.

There are also alternatives to steroid injection such as Ostenil (hyaluronic acid) which may well be just as effective, have less side effects and last longer.

Sodium hyaluronate (also known as hyaluronic acid or hyaluronan) is a natural substance. It is present in the body wherever moisture is stored or lubrication between layers of tissue is required to eliminate friction. Examples are the vitreous body of the eye itself, the tear film, the joint cartilage, the synovial fluid in the joints, all the mucuous membranes of the body, but also the basic substance of the skin which consists of up to 55% sodium hyaluronate. Sodium hyaluronate takes on a variety of functions:

  • Storage: it absorbs and stores moisture in enormous quantities
  • Lubrication
  • Transport medium for nutrients
  • Filter for inflammatory molecules.

It may help to visualise sodium hyaluronate as a dish of spaghetti (this is what the molecules look look like under an electronic microscope): a dense mesh that remains elastic and flexible in spite of its density and interwoven structure.

How does sodium hyaluronate work?

Sodium hyaluronate is an essential component of the synovial fluid. It enables the fluid to act as a lubricant, a shock absorber and a filter controlling the movement of cells and large molecules within the joint:

  • Under gradual shear stress, sodium hyaluronate acts as a lubricant
  • Under sudden loading, sodium hyaluronate acts as a shock absorber
  • Sodium hyaluronate acts as a filter, hindering the movement of potentially damaging cells and molecules

Steroid is a prescribed medication used to reduce inflammation. It lasts 3 months giving a window to get pain free. It is a very powerful anti inflammatory and therefore painkiller however in terms of joint protection it has little mechanical effect on the joint and won’t directly protect against further wear & tear. Once pain free the muscles will work better and control the joint better.

Ostenil is a made of hyaluronic acid, a naturally occurring substance in the body. it is designed o aid th joint in a more mechanical way and helps to lubricate and cushion joints which then helps to reduce inflammation and pain. It therefore helps to prolong a joints viability protecting potentially protecting against wear whilst also offering pain relief and lasts 6-9months.

Whilst both can also be used to address tendon conditions careful consideration is required as to the best option.

In short, yes.

To obtain the highest accuracy and and therefore best result ultrasound is used to assess any issue present and then aid injection whether it be indirect (used to accurately mark an injection site) or direct whereby ultrasound is used to guide the needle directly in realtime and monitor the introduction of the injectable.

  1. An injection of sodium hyaluronate increases the viscosity of the synovial fluid, restoring its lubricating, shock-absorbing and filtering properties.
  2. In addition, it re-establishes the protective coating of hyaluronan over the inner surface of the joint and increases the scavenging of free radicals.
  3. As a result of these changes, sodium hyaluronate reduces inflammation of the synovial membrane.

The direct effects of adding hyaluronan (i.e. injections of artificial sodium hyaluronate) cannot account for its long-term benefits, as it is cleared from the joint within a few days. It appears that, through its direct effects, exogenous hyaluronan restores the ability of the joint to produce its own hyaluronan and thus returns it to a state of homeostasis (i.e. stable condition) that persists for several months.

Evidence exists suggesting that exogenous hyaluronan may slow the destruction of cartilage

The improvement in your symptoms should last for several months, depending partly on the number of injections you have. You can expect the benefit from three injections to persist for up to 9 months or from a single Ostenil Plus 6-9 months.

You may not notice any benefits immediately after the first injection, but you should gradually start to feel less pain and stiffness over the next few weeks.

All OSTENIL® products are licensed for use in any synovial joint of the body, typically the knee, hip, or shoulder joints. Ostenil® products are also suitable for the elbow, ankle, and some of the small joints such as the jaw, hand (finger, wrist and saddle joints) and toe joints.

OSTENIL® does not contain any animal proteins, so it is fine to be used for vegetarians.
